This new service aims to provide a qualified response in the home context, in terms of health care and well-being, offering the possibility for those who wish to grow old at home, with comfort, safety and autonomy.
Interventions with users are personalized according to their specific needs. Supported by a multidisciplinary team, a care plan defined by the family doctor is followed and, with the use of technological systems, it allows the identification of deviations that are automatically communicated to the health provider.
