The main objective of IIES Saúde em Casa 2.0 is to create and implement an innovative care solution.
This objective aligns with the growing need to address the vulnerabilities faced by the 65+ population, such as social isolation, limited access to healthcare, and lack of adequate support, given the national trend of population aging. The proposed solution aims to improve the quality of life and well-being of seniors by providing personalized and continuous care directly in their homes, using a multidisciplinary team and technological support.
This integrated model combines remote health monitoring, efficient medication management, cognitive and social stimulation, and training for informal caregivers, aiming to prevent health complications, reduce hospitalizations, and foster the autonomy and dignity of seniors.
